The Intergenerational Wellness Support Initiative aims to engage students in nursing, social work, or allied health to provide community-based wellness support to older adults. The project focuses on delivering structured case management services that align with Tellegacy’s intergenerational and wellness-centered approach. Students will conduct comprehensive assessments to identify the health and wellness needs of older adults. They will provide personalized health education, empowering older adults to make informed decisions about their health. Additionally, students will contribute to care planning, ensuring that the plans are tailored to the unique needs of each individual. This project offers students the opportunity to apply their classroom knowledge in real-world settings, enhancing their skills in assessment, education, and care planning.

Deliverables

The project deliverables include a comprehensive assessment report for each older adult participant, detailing their health and wellness needs. Students will also create personalized health education materials tailored to the specific needs of the older adults they work with. Additionally, students will develop a care plan for each participant, outlining strategies for maintaining and improving their health and wellness. These deliverables will demonstrate the students' ability to apply their academic knowledge to practical, community-based scenarios.

Why Tellegacy was started. After finding out from a community Chaplain friend that older adults in retirement communities were passing away, not from a pandemic, but from an epidemic of loneliness and a broken heart due to isolation from family and friends, Jeremy Holloway, PhD, decided to create a flexible, relationship-oriented mindfulness, goal setting and guided imagery curriculum that can be facilitated by University students via phone or virtually one-on-one with older adults in retirement communities. Tellegacy program make meaningful intergenerational connections, we think it can save lives.
