diabetes mellitus dementia relationship building social determinants of health chronic diseases communication constructive feedback health equity usability testing community healthThe African American/Black (AA/B) older adult population is projected to triple and will account for 12.8% of older adults by 2060. Additionally, they are disproportionately affected by chronic conditions such as dementia (up to twice), hypertension (75% versus between 40-55% for White men and women), and diabetes (13.2% versus 7.6% Whites in one study). Risk factors such as lack of access to healthcare, education, transportation, and living in distressed neighborhoods (i.e., social determinants of health) have led to poor health outcomes and health inequities.
One approach using community health workers (CHWs) shows effectiveness across multiple settings and conditions for bridging the gaps in health disparities by building trust, improving screening and referral processes, and expanding communication and health information delivery to AA/B older adults. However, scaling effective CHW programs through continuous enrollment and engagement needs to be improved to address the increasing caseloads and limited funding.
Benten Technologies is developing an innovative platform called Expanding Research and Recruitment by Engaging Adults through Community Health Workers (eREACH) that supports CHWs in the process of screening and referral and in engaging AA/B older adults in local health services, clinical trials, and health events. Innovative features of eREACH include electronic screening, an integrated patient dashboard, microlearning education, and enhanced communication via a virtual agent named “Reggie”.
Project Scope
We have successfully developed low-fidelity wireframes for eREACH, catering to three distinct end users: CHWs, AA/B older adults, and their trusted family and friends (TFFs). To take our design to the next level, we are looking to collaborate with SpringBoard designers to enhance our existing low-fidelity wireframes and transform them into high-fidelity/interactive wireframes for user testing and development.
High-Level Requirements
- Enhance and complete the existing low-fidelity wireframes for three end users: CHWs, AA/B older adults, and their TFFs
- Transform low-fidelity wireframes into high-fidelity wireframes
- Provide design system for current and future expansion of the program including style guides
Preferred Team Member experience
- Team member has experience in wireframes design
- Team member has experience in educational domain or related project in area
- Team member with experience in concept design and creative thinking
Deliverables
- Improved Low-fidelity Wireframes in Figma
- Interactive Wireframes in Figma

High-Level Timeline/Schedule
- Week 1 – Onboarding, Current App Design Walkthrough, IDP Goals, Finalize / Align IDP Plan
- Week 2 – Enhancing low-fidelity wireframes
- Week 3 – Creating interactive wireframes
- Week 4 – Creating interactive wireframes and testing with target audience
- Week 5 – Revising interactive wireframes based on user feedback
- Two meetings per week
- Status/Q&A Meeting (15 minutes): Schedule early in the week (Monday or Tuesday). This brief meeting will be held to discuss the goals for the week and address any questions or concerns. It will serve as an opportunity for everyone to stay updated on the progress and ensure alignment.
